# Completing the report

## General information about the evaluation

The purpose of completing this section of the questionnaire is to gather the information related to the evaluation in order to ensure its monitoring and traceability.

Below are the instructions for completing each field of the report.

<summary>Identity of the members of the evaluation team and affiliated organisations</summary>

Information regarding the identity and membership of an entity in the evaluation team, in order to assess the objectivity and legitimacy of the evaluation.

The information to be provided for each team member, **as part of the evaluation of a Bilan Carbone®**, is as follows:&#x20;

* surname
* first name
* qualifications
* the organisation(s) to which they belong and their positions
* the [required skill(s)](https://www.bilancarbone-evaluation.com/guide-devaluation-des-bilans-en/process/the-evaluation-team#composition-de-lequipe-devaluation-dun-bilan-carbone-r) to which they meet

**As part of the evaluation of a regulatory GHG Assessment** : identification of the DREAL verifying the assessmen&#x74;*.*

<summary>Certification of the evaluation project lead (and, where applicable: identification number of the chartered accountant participating in the evaluation)</summary>

The evaluation project lead is [certified for evaluation](https://www.bilancarbone-evaluation.com/guide-devaluation-des-bilans-en/resources/bibliography#certification-a-levaluation-des-bilans) : enter their certification number in the field and attach proof (certificate) to the report.

If a chartered accountant participates in the evaluation process, enter their identification number.

SIREN number of the legal entity or organisation to which the assessment relates, as well as all SIRET numbers identified under this SIREN.

Where the person carrying out the assessment is an external organisation or individual to the organisation, specify:&#x20;

This is requested for monitoring purposes and to distinguish between the person carrying out the assessment and its evaluator.

<summary>Temporal boundary and publication year of the assessment</summary>

This data makes it possible to understand the time frame of the information presented and assess how up to date it is.

The temporal boundary (or reporting year in the case of BEGES-R) of the assessment is the period covered by the activity data collected to prepare the assessment. The publication year is the year in which the assessment was published in the database [OCCF](https://www.bilancarbone-evaluation.com/guide-devaluation-des-bilans-en/resources/bibliography#occf).
